A medical device rep at the median earns about $175,000 on target, and the best-paid implant and endoscopy roles run past $400,000. The spread between those numbers is the story: base pay ranges from zero to six figures, variable pay is half or more of total earnings, and quota attainment swings from under 30% in some roles to 80% in others. Total compensation and base salary.
Medians and averages disagree because the sources measure different populations: RepVue's verified submissions skew to large-company field reps with heavy variable pay, MedReps surveyed all medical sales, and the Bureau of Labor Statistics counts every technical product rep in the country. Read them together and the picture is consistent: a six-figure median, a long upper tail, and a base salary that varies more than total pay.

Pay by specialty, product and employer.
Specialty and employer move device pay more than years of experience do. Capital equipment and cardiovascular roles carry the highest on-target earnings, implant roles at the large orthopedics companies pay the most at the top of the range, and the same job title can pay twice as much at one employer as at another.

Commission, quota and variable pay.
Roughly half of a device rep's income is at risk every year, and in several large-company implant roles all of it is. These figures cover how often reps make quota, what share of pay is variable, and the commission rates recruiters and comp vendors quote for W-2 and 1099 roles.

RepVue's 66.5% is the share of verified device reps reporting quota in the trailing year; its industry-wide 52% average and Alexander Group's cross-industry 49% describe different populations. All three say the same thing: device reps hit quota more often than B2B sellers in general, and about a third still miss.
Experience, age and demographics.
Pay in medical sales climbs steeply with experience and peaks in a rep's forties. The workforce remains majority male and the measured pay gap, last quantified by MedReps in 2015, has not been publicly re-measured for device sales since.

Hiring, turnover and time to ramp.
The official outlook for technical product reps is flat headcount with heavy replacement hiring. Sales turnover has fallen from its 2022 peak. Ramp time for device reps is quoted everywhere and measured almost nowhere: the training programs at the largest manufacturers run nine to fourteen months, which is the most concrete public benchmark.

The job itself: access, time and satisfaction.
The defining change in the device rep's job is access. Clinicians and administrators increasingly prefer virtual or mixed-channel contact, face-to-face is now a minority of the channel mix, and administrative work absorbs hours every week. Despite that, reported job satisfaction stays high, and the reps using AI tools report meaningfully better quota results.

Treat the direction as solid and the exact percentages as indicative.

Purchasing concentration is the one to watch. With nearly every hospital buying through a GPO and every surveyed hospital running a value analysis committee, the individual rep's influence runs through committees as much as through surgeons. That is the selling environment behind the attainment numbers above.

Device sales pays more than pharma on total earnings and less in base; it pays about the same as enterprise SaaS on target but with a far higher share of reps actually attaining quota. These comparisons use the same sources as the rest of the page so the numbers are like for like.

Nine to twelve months is the commonly quoted range, though it comes from vendor rules of thumb rather than a published survey. The pay and satisfaction data say yes with caveats. Figures from the 2021 MedReps survey are the most recent public edition; MedReps has not published a public report since, and the "2023" and "2024" MedReps numbers circulating on aggregator sites could not be traced to any MedReps publication and are excluded.
